As a licensed childcare center, we are required to collect the following information for each child before the fun can begin.  Please complete the following forms before your first visit and bring them with you.  After the initial enrollment, check-in/out will be a breeze with our fingerprint technology!

1) Print and complete our Admission Form

2) Bring copies of the following:

• Immunization Records (if not on file at another daycare or school)
• Statement of child’s health from a health-care professional (please see Admission Form for more details)

If your child will need medication administered while at Ashley’s Playhouse:

If your child has a food allergy, parents need to submit  a Food Allergy Emergency Plan signed by their healthcare professional.

Online Reservations

Please note, you will need to create a NEW username/password for Daysmart Appointments the first time you book online.  If you do not see a spot available online, please call or e-mail us to check for availability.  We are still asking for reservations for all age groups and times due to limited staffing.
